              I am the applicant identified on the Application Form. I hereby apply for membership, as indicated on the Application Form, in Sunset Speedway Motorsports Limited (herein known as S.S.M.L.) and, authorize S.S.M.L. to represent my interests as they may appear in auto racing matters. I acknowledge that I am familiar with the current rules of S.S.M.L. and agree to abide by the current rules of S.S.M.L. as they may be amended from time to time. I understand that this registration is not transferable and may be suspended by S.S.M.L. pursuant to the provisions of the S.S.M.L. Rule Book as it may be amended from time to time.
           
              I further certify that I am an independent contractor and not an agent, servant or employee of S.S.M.L. and that I will retain such status as an independent contractor in the event my membership and registration application is approved by S.S.M.L. . I acknowledge and assume all responsibility for any charges, record keeping, premiums and taxes, if any, payable on any funds I may receive as a result of my activities as a S.S.M.L. member, including but not limited to, unemployment insurance taxes, workers compensation insurance, income taxes and withholding taxes.
           
              ADVERTISING AND PROMOTION RELEASE: S.S.M.L. or its duly authorized agents and assigns, may use the APPLICANT'S name, likeness and photographs of the APPLICANT or the APPLICANT'S cars, including photographs of the APPLICANT or the APPLICANT'S taken during the EVENT, in any way, medium or material, for promoting, advertising, recording or reporting any S.S.M.L. sanctioned EVENT (S) before, during and after such EVENT (S), including but not limited to television and radio broadcasts, film production, videotape reproductions and the like, and do hereby relinquish all rights thereto for these purposes.
           

I understand that acceptance of this membership application and fee by any S.S.M.L. Official does not constitute approval of this application and that all applications must be approved by S.S.M.L. Headquarters ..... P.O. Box 7153 Innisfil, Ontario, L9S 1A9
